Implant abutments serve as the critical biomechanical and biological interface between the osseointegrated fixture and the prosthetic crown. As digital dentistry has expanded access to computer-aided design and computer-aided manufacturing (CAD/CAM) technology, individualized customized abutments have become an increasingly viable alternative to conventional prefabricated stock components. This narrative review examines the comparative performance of customized and standard abutments across four domains: biomechanical behavior, peri-implant tissue response, esthetic outcomes, and digital workflow integration. Current evidence suggests that customized abutments offer meaningful advantages in emergence profile management, soft tissue conditioning, peri-implant mucosal stability, and anterior esthetic outcomes. Biomechanical performance appears comparable between both types when appropriate materials and connection geometries are selected. Long-term randomized clinical trials using standardized outcome criteria remain scarce; therefore, definitive superiority cannot yet be established. Future research should incorporate standardized reporting frameworks to improve comparability across studies.
