Solid Pseudopapilary Neoplasm (SPN) are rare tumors comprising only 1 to 3% of all pancreatic tumors with a clear female of younger age preponderance. Presentations beyond 40 are extremely rare. We describe the case of a 52 years old patient who complained of persistent upper abdominal pain and was eventually found to have SPN of the uncinated pancreatic process. A neuroendocrine tumor (NET) was suspected even before surgery, as pancreatic NET and SPN frequently pose diagnostic challenges. The gold standard treatment is surgical resection, and while the prognosis is frequently excellent, there is risk of metastatic disease and recurrence. Close follow up of the patients after surgical resection is advised.
